Catholic Ministry Formation Register
The Catholic Ministry Formation Register (CMFR) is intended to keep a record of those in formation for Clerical or Religious Ministry.
The CMFR will aid in the screening of applicants and serve as a historical record of formation. Deidentified aggregate data from this system relating to safeguarding will help inform ACSL in coming years.
Below you will find information about the Register, and the forms that Church entities, Contact People and those being registered will use to engage with us.

Forms:
For Church Authorities to use in registering their Entity to participate in the CMFR and nominate a Contact Person.
For Contact People and those applying for formation to complete to enter them onto the Register.
For Contact People and those already in formation to complete. Please note each Church entity is free to decided whether to register those already in formation when joining the CMFR.
To be used by the Church Authorities of entities which are already participating to name a new Contact Person.
